Creating an image window on the root page in Menu Designer

Hello All,

I seem to only have questions about creating and modifying menus and this is no different.

I've noticed that amost every menu template has a place holder on the root menu page for an image or video (labeled 1). However, when I want to create my own custom template and go to Menu Designer, there seems to be no way to set the same placeholder for an image or video. The Add Button icon adds "item 4" but no image window. On the Titles/Chapters page, the Add Button adds not only "item 2" (or higher) but an attached image window also. Why isn't this being done on the Root Menu page? How are the menu template designers doing this? Plus, as an related observation, if I import one of the templates and switch to Modify, I can't delete the image placeholder from the Root Menu page. What's the trick? Thank you.
