why do transition effects keep getting reversed?

fades and other transition effects keep becoming reversed for no reason. and then instead of fading out, it abruptly ends and fades IN... any idea why?

This has become a regular 'feature' in PD since PD10 and still does it in PD12

I have no idea why it does it. It has been mentioned here before.

The only way I have found to prevent it from spoiling your production is to close out of PD and the re-open and produce your project.

A pain, I know!

Is it possible that the position of the fade effect has been inadvertently altered?

There are three choices in most situations...

1. Effect placed ahead of the split: scene one fades out, then the next shot pops on
2. Effect straddling the split: scene one fades out as scene two fades in
3. Effect after the split: scene one pops out, then scene two fades on

To add to the above- expand your time line and check to make sure there isn't another fade where it shouldn't be. (Sometimes they are hard to see unless you drag all the way out and see them.)

Also double click on your clip or click the Modify tab that is causing you issues.
Scroll all the way down in the modify window and make sure these aren't checked:

All other Programs have two seperate fades you can manually place to stop this happening. Seems simple enough. The fade reversal is a stupid problem. It needs to be fixed.
