The frames, sometimes before, sometimes after, the transition, either go in slow mo or really fast, or they are really choppy. Is there something I am doing wrong with the transition itself that is causing this?

Try this first...... if you have not already....
Set your preview resolution lower. PD renders your preview on the fly and on a LOT of computers it bogs down on transitions and effects. Even though you watch it at a lower resolution it will render/produce fine and the transitions and effects will be smoother. I have a decent computer but I still use the normal setting. Since I have used it for so many years... I don't need to see it in crisp clear HD preview. You wont either after a while...
